| 目的 在观察针刺治疗偏头痛的临床疗效基础上,从神经影像学角度探讨其对脑功能网络的影响。方法 选取我院2018年1月～2019年11月期间收治的偏头痛患者250例为研究对象,随机分为对照组(125例)和观察组(125例)。对照组采用盐酸氟桂利嗪治疗,观察组在此基础上给予针刺治疗。比较两组治疗前后头痛频次、知觉压力、VAS评分、睡眠质量及生活质量情况。采用静息态功能磁共振扫描对两组患者脑功能连接网络的变化情况进行观察。结果 治疗后,两组头痛频次减少、知觉压力及VAS评分明显下降,睡眠质量明显提高,且观察组更明显(P<0.05)。治疗后,两组在功能受限、功能障碍及情感各领域评分明显提高,且观察组更明显(P<0.05)。治疗后,观察组的功能连接网络表现出动态的功能连接性增强以及功能连接脑区增多,而对照组的功能连接网络表现为功能连接度的动态减弱和功能连接脑区减少。结论 针雌对治疗慢性偏头痛具有明显效果,改善其临床疼痛症状,并对其的功能连接网络有着动态的调整和修饰作用。 |

| Objective To observe the clinical effect of acupuncture on migraine and explore its influence on brain functional network from the perspective of neuroimaging. Methods 250 patients with migraine in our hospital from January 2018 to November 2019 were randomly divided into control group (125 cases) and observation group (125 cases). The control group was treated with flunarizine hydrochloride, and the observation group was treated with acupuncture on this basis. Headache frequency, perceived pressure, VAS score, sleep quality and quality of life were compared between the two groups before and after treatment. Resting state functional magnetic resonance imaging was used to observe the changes of brain functional connectivity network in the two groups. Results After treatment, the headache frequency, perceived pressure and VAS scores of the two groups were significantly decreased, and the sleep quality was significantly improved, especially in the observation group (P < 0.05). After treatment, the scores of functional limitation, dysfunction and emotion in the two groups were significantly improved, and the observation group was more obvious (P < 0.05). After treatment, the functional connectivity of the observation group increased and the functional connectivity increased, while the functional connectivity of the control group decreased. Conclusion Acupuncture has obvious effect on treating chronic migraine, improving its clinical pain symptoms, and dynamically adjusting and modifying its functional connection network. |
