      摘要:目的:研究加味涤痰汤联合双水平无创正压通气对慢性阻塞性肺疾病急性加重(AECOPD)合并Ⅱ型呼吸衰竭患者血气分析及右心室收缩压的影响。方法:选取2022年4至2024年3月浙江省台州市中医院呼吸与危重症医学科收住的AECOPD合并Ⅱ型呼吸衰竭患者70例,按照随机数字表法分为对照组34例和治疗组36例。对照组予双水平无创正压通气及西医常规治疗,治疗组在对照组的基础上加用加味涤痰汤,连续治疗1周。比较治疗后两组患者的PH值、氧分压(PaO2)、二氧化碳分压(PaCO2)、右心室收缩压(RVSP)数值。结果:治疗后治疗组PH值、PaCO2数值下降优于对照组,存在统计学意义(P<0.05);两组患者PaO2在治疗后均明显改善,但两组间治疗后的PaO2不存在统计学意义(P>0.05);治疗组RVSP数值较前下降但不存在统计学意义(P>0.05)。结论:加味涤痰汤联合双水平无创正压通气能够有效改善患者缺氧状态、减轻二氧化碳潴留,纠正呼吸性酸中毒,并在一定程度上对右心室收缩压获益。

      Abstract: Objective: To study the effects of Jiawei Ditan Decoction combined with double-level non-invasive positive pressure ventilation on blood gas analysis and right ventricular systolic pressure in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) complicated with type II respiratory failure. Methods: A total of 70 patients with AECOPD combined with type II respiratory failure admitted to the Department of Respiratory and Critical Care Medicine, Taizhou Hospital of Traditional Chinese Medicine, Zhejiang Province from April 2022 to March 2024 were selected and divided into control group (34 cases) and treatment group (36 cases) according to random number table method. The control group was treated with double-level non-invasive positive pressure ventilation and standard Western medicine treatment, and the treatment group was treated with Jiawei Diitan Decoction for 1 week. The PH, PaO2, PaCO2 and RVSP values of the two groups were compared after treatment. Results: After treatment, the PH value and PaCO2 value of the treatment group were lower than that of the control group, with statistical significance (P<0.05). PaO2 in both groups was significantly improved after treatment, but there was no statistical significance in PaO2 in both groups after treatment (P>0.05). The RVSP value of the treatment group was decreased, but there was no statistical significance (P>0.05). Conclusion: Jiawei Ditan Decoction combined with double-level non-invasive positive pressure ventilation can effectively improve hypoxia, reduce carbon dioxide retention, correct respiratory acidosis, and to a certain extent benefit the right ventricular systolic blood pressure.
