      目的:探讨张氏手法整复联合“三维抗旋转”固定对伸直型儿童肱骨髁上骨折的临床疗效。方法:收集2021年1~2024年1月在杭州市富阳中医骨伤医院张绍富骨伤研究所纳入伸直型儿童肱骨髁上骨折患儿60例,依据随机数字法分试验组以及对照组,各30例。对照组采取“三维抗旋转”固定,试验组在对照组的基础上予以张氏手法整复。对比两组临床疗效、随访时间、骨折愈合时间、肘内翻畸形发生率、ROM度数及生活质量。结果:试验组优良率为83.33%略高于对照组优良率73.33%,但统计学检验显示无差异(P>0.05)。与对照组相比,试验组手术时间较长,骨折愈合时间缩短,肘内翻畸形发生率降低(P<0.05),随访时间相比无统计学差(P>0.05)。与对照组比较,试验组肘关节屈肘、伸肘、过伸ROM度数较高(P<0.05)。疗后所有入选者各项GQOLI-74评分较疗前增高(P<0.05);与对照组进行对比,试验组各项GQOLI-74评分较高(P<0.05)。

      Objective: To investigate the clinical effect of Zhang""s manual consolidation combined with "three-dimensional antirotation" fixation on supracondylar humeral fractures in extended children. Methods: 60 children with extended supracondylar humeral fracture were collected in Zhang Shaofu Research Institute of Fuyang Traditional Chinese Medicine Bone Injury Hospital, Hangzhou from January 2021 to January 2024, and 30 cases were divided into the test group and the control group according to the random number method. The control group adopted "three-dimensional anti-rotation" fixation, and the test group gave Zhang""s technique rehabilitation on the basis of the control group. Clinical efficacy, follow-up time, fracture healing time, incidence of ancubital valgus deformity, ROM degree and quality of life between the two groups. Results: The excellent rate of 83.33% was slightly higher than that of 73.33%, but the statistical test showed no difference (P> 0.05). Compared with the control group, the test group had a longer operation time, shorter fracture healing time, reduced incidence of ancubital valgus deformity (P <0.05), and no worse follow-up time (P> 0.05). Compared with the control group, the ROM degree of elbow flexion, elbow extension, and hyperextension in the test group was higher (P <0.05). GQOLI-74 scores increased (P <0.05); GQOLI-74 scores were higher compared with the control group (P <0.05). Conclusion: Zhang""s reduction technique is combined with "three-dimensional anti-rotation" fixation method in the treatment of extended children, and the curative effect is remarkable. This method promotes fracture healing and ensures the safety of the treatment process.
