| 【】目的:研究探讨四物五子汤化裁联合氩激光治疗肝肾亏虚型糖尿病性视网膜病变(DR)II~III期的临床疗效。方法:将2021年6月至2023年6月间收治的150例肝肾亏虚型II~III期DR患者,随机分2组,各为75例(150眼),两组均给予常规降压、降脂、控制血糖干预,对照组在此基础上给予氩激光治疗,观察组在对照组基础上加用四物五子汤化裁治疗,比较两组中医症候积分,眼底、视力情况,黄斑病变情况,眼部血流动力学指标及疗效效率。结果:两组中医症候积分,眼底、视力情况,黄斑病变情况,眼部血流动力学指标组间均不存在统计学意义(P＞0.05)；治疗1个月后,两组各症候积分分值、病灶隆起最高点视网膜平均厚度(HLT)、病灶区视网膜平均厚度(CLT)、视网膜平均厚度(CMT)、最佳矫正视力(BCVA)、黄斑厚度、出血斑面积检查值均明显减小(P＜0.05),30°内阈值敏感程度、舒张期血流速度(EDV)、收缩期眼动脉的最大血流速度峰值(PSV)检测值均明显增大(P＜0.05),观察组改善幅度更显著(P＜0.05)。两组疗效总有效率分别为93.33%、78.67%,组间存在统计学意义(P＜0.05)。结论:肝肾亏虚型II~III期DR患者采用四物五子汤化裁联合氩激光治疗的疗效显著,优于单用西医治疗,有效改善视野和视力,改善眼部血流动力学,减轻黄斑病变程度。 |

| Objective: To study the clinical efficacy of Siwu Wuzi Decoction combined with argon laser in the treatment of stage II to III diabetes retinopathy (DR) with liver and kidney deficiency. Method: A total of 150 patients with liver and kidney deficiency type II-III DR admitted between June 2021 and June 2023 were randomly divided into two groups, each consisting of 75 cases (150 eyes). Both groups received routine blood pressure lowering, lipid-lowering, and blood glucose control interventions. The control group received argon laser treatment on this basis, while the observation group received modified treatment with Siwu Wuzi Tang in addition to the control group. The TCM symptom scores, fundus and visual acuity, macular lesions, ocular hemodynamic indicators, and therapeutic efficacy of the two groups were compared. Result: There was no statistically significant difference (P>0.05) in the scores of traditional Chinese medicine symptoms, fundus and visual acuity, macular degeneration, and ocular hemodynamic indicators between the two groups; After 1 months of treatment, the scores of various symptoms, mean retinal thickness (HLT) at the highest point of lesion protrusion, mean retinal thickness (CLT) in the lesion area, mean retinal thickness (CMT), best corrected visual acuity (BCVA), macular thickness, and bleeding spot area in both groups were significantly reduced (P<0.05). The sensitivity of the 30 ° threshold, diastolic blood flow velocity (EDV), and peak systolic blood flow velocity (PSV) of the ophthalmic artery were all significantly increased (P<0.05), and the improvement in the observation group was more significant (P<0.05). The total effective rates of the two groups were 93.33% and 78.67%, respectively, with statistical significance between the groups (P<0.05). Conclusion: The treatment of stage II-III DR patients with liver and kidney deficiency type using Siwu Wuzi decoction combined with argon laser is significantly more effective than using Western medicine alone, effectively improving visual field and vision, improving eye hemodynamics, and reducing the degree of macular lesions. |
